A former project manager in the nuclear industry, James Titcombe became involved in patient safety following the tragic loss of his baby son due to failures in his care at Morecambe Bay Trust in 2008.
Working with other families, he sucessfully campaigned for the Morcambe Bay Investigation which was published in March 2015. In 2013 he began work as the National Advisor on Safety for the Care Quality Commission. James is now a patient safety specialist for Datix.
He regularly speaks at national conferences on the subject of patient safety and has had articles published in healthcare journals as well as the national press.
James is a Patron of the charity Action against Medical Accidents (AvMA).
In 2015 James was awarded an OBE for services to patient safety.
